Melvin Cook, 40 of Dade City, went home to be with his Lord and Savior, Jesus Christ, on October 4, 2014. He was born on January 10, 1974 in Gallipolis, Ohio to Charles and Lucinda Cook. He is survived by his wife, Christy Cook, his children, Garret Cook (15) and Gavin Cook (6), his father Charlie Cook, brother Glenn Cook, sisters, Percilla Cook, Carlena Cook and Katrena Cook.A visitation will be held on Saturday, October 18th, from 11:30am to 1:00pm, at Hodges Family Funeral Home 301 chapel in Dade City, with a funeral service at 1:00. Melvin will be laid to rest in Chapel Hill Gardens cemetery.
